Care at Home is an overarching term we use to simplify a variety of services offered by either Black River Memorial Hospital or the Black River Healthcare Clinic. The services below can be used either alone or in conjunction with another to further individualize your care to your specific needs.

The outpatient Physical Medicine and Rehabilitation department offers in-home therapy options to those unable to leave the home due to a documented medical reason.
The Home and Respiratory Equipment department offers home equipment such as BIPAP, CPAP, and other respiratory items to qualifying patients. This includes 24/7 staff coverage for oxygen needs and safety. This is provided under our Durable Medical Equipment Accreditation.
Supportive care services are available to residents of Jackson County. These services are completed by a trained supportive care worker and are limited to bathing, oral cares, incontinence care, hair grooming, ambulation/repositioning, or feeding assistance.
Please note: Supportive care is unlicensed and uncertified with no provider orders or registered nurse oversight. This is a cash-based service menu selected by the resident themselves.
Patient and nurse work together to arrange patient care, communicate healthcare needs and concerns among all healthcare providers involved in the patient's care, with the goal of increasing safety at home. This service is through a combination of in-person clinic visits and telephone conversations.
Examples of chronic conditions: heart disease, Chronic Obstructive Pulmonary Disease (COPD), diabetes, high blood pressure, and arthritis.
